Insurance and Financial Information for the Patient
Insurance
When you begin treatment with us, we will help you find out what your benefits are and also provide you with claim forms. We will be happy to help you maximize your benefits, however, it is the patient’s responsibility to file their own insurance claims and obtain reimbursement. You can expect your insurer to reimburse you for a portion of our fee. If your insurance company needs a report and x-rays, we will send them off promptly.
Payments
We accept checks, cash, Visa, MasterCard, American Express, Discover, CareCredit and Green Sky financing. Payment is due at the time service is rendered unless other arrangements have been made in advance.
